PSW’s may support the client with:
Showering or bathing and assistance dressing
Toileting
Medication reminders
Assistance with safe transfers and mobility
Meal preparation
Social support
Assistance with routine, light household activities such as sweeping, dusting, dishwashing, vacuuming, laundry
Eligibility:
Client has PSW supports in place from Home & Community Care Services
A Matthews House Hospice volunteer is unable to meet the client’s care needs
Client has a life-limiting illness (PPS score is 50% or less)
Client requires assistance with Activities of Daily Living
Responsive or defensive behaviors (known to inflict physical harm to caregivers) must be managed before a PSW can provide services
Client is not currently living in a retirement home or congregate living setting
Client has a safe home environment with a clear pathway for PSW’s to access the home
Services for housekeeping tasks must be in the client’s personal living area
Frequency:
Matthews House can provide 4 hours/week (2hrs, twice/week and on occasion for caregiver relief, 4hrs, once/week). PSW support is available between 9:00am and 5:00pm, 7 days per week. Clients will be reassessed every 3 months to determine ongoing eligibility.
